| Implementation Period: | 1 April 2019 to 15 January 2020 |
| Eligibility: |
1. “Healthy City” Organisations / Associations in the Southern District
2. Local Bodies / Organisations in the Southern District
3. Non-governmental Organisations (NGOs)
The above organisations which meet the following criteria are considered eligible for submitting applications –
(a) a statutory organisation or a body / organisation registered under the laws of the Hong Kong Special Administrative Region (e.g. the Companies Ordinance (Cap. 622), the Societies Ordinance (Cap. 151), and the Inland Revenue Ordinance (Cap. 112)) and established wholly or mainly for the benefit of the district in which it is set up. For territory-wide organisations established to serve the public as a whole, their application should involve a project that will benefit the local community and the people who live, work or study in the district; or
(b) an autonomous body / organisation of persons (whether incorporated or not) established wholly or mainly for the benefit of the district in which it is set up; or an organisation established to serve the general public which organises activities for the benefit of the local community and the people who live, work or study in the district.

| Project Theme and Nature: |
1. To achieve the objectives of “Towards 2025: Strategy and Action Plan to Prevent and Control Non-communicable Diseases in Hong Kong”, and implement activities / projects for health promotion based on the health needs in the actual context of the Southern District.
2. All funded activities / projects must contain public education on – the interrelationship between the four main types of non-communicable diseases (namely cardiovascular diseases, cancers, chronic respiratory diseases and diabetes) and the four shared behavioural risk factors (namely unhealthy diet, physical inactivity, tobacco use and alcohol consumption) with a view to facilitating prevention of diseases (Sample Contents to be provided by the Department of Health).
3. The activity / project should include one or more of the above behavioural risk factors:
(a) Choice of healthy diet – eat food that is low in oil, salt and sugar, cook dishes with low salt, low sugar and little oil, plus an intake of at least two servings of fruit and three servings of vegetables per day;
(b) Increase daily exercises – adults should engage in moderate-intensity physical activities for at least 150 minutes a week; children and youth (aged 5 – 17) should engage in moderate- to vigorous-intensity physical activities for at least 60 minutes per day;
(c) No smoking;
(d) No alcohol.
4. The activity / project should comprise all of the following components:
(a) To implement a pro-health policy – leading adaptation on the policy level such that upon completion of the activity, the healthy behaviour of the target group can keep going;
(b) To mobilise cross-sectoral organisations, framework and administrative system – implementation of the health policy should require the collaborative effort of all organisations, framework and administrative system;
(c) To create favourable condition in the environment in support of healthy choice and behaviour – the health policy should ensure that the living environment and atmosphere are favourable for healthy behaviour;
(d) To encourage community engagement – mobilise participants of the target groups to take part in the discussion, design and implementation of the activity / project so that the contents can suit their needs better; and
(e) To enhance the participants’ health awareness and technique of a healthy lifestyle – through learning and cognition, enable the target groups to recognise that small changes in the daily life can improve their health.

| Public Engagement: |
Applicant organisations must include public engagement elements in each of the two activity stages as follows:
1. Activity Plan and Design Stage
To enable public discussion of the activity theme and proposals by organising a workshop / seminar / focus group, issuance of open invitation and organising competitions to solicit activity proposals from the communities of the Southern District, seeking public opinions via different publicity channels such as online platform and questionnaire survey.
2. Selection of Final Proposal Stage
Upon collection of public proposals, the selection of the final version of the proposal for activity plan should include the means of public engagement. The selection should be mainly by means of public voting. Other ways to engage public participation in the selection process such as through online channels are also favourable to the determination of the final version of the proposal for activity plan.

| Application Deadline and Arrangement: |
1. Please submit the following documents to the SDC Secretariat on or before 4 February 2019 (Monday) upon:
(a) Completing the Promotion of “Towards 2025: Strategy and Action Plan to Prevent and Control Non-communicable Diseases in Hong Kong” Community Funding Scheme - Information of proposed activity / events (Application Form) (i.e. Annex 3) enclosed with the Gist of the Scheme and Funding Criteria; and
(b) Making a proposal to specify how the abovementioned public engagement elements are to be included.
2. Late applications will not be considered.
